Age UK Portsmouth plays a vital role within the community to reduce the pressure on the NHS emergency and acute hospital services over this difficult period.
AgeUK Portsmouth can provide support during the rehabilitation period for those recently discharged from hospital and to those most at risk of a hospital admission.
The Age UK Portsmouth Covid Surge Support programme project is designed to support users who meet the first point and at least one other criteria below:
ESSENTIAL
Been discharged from hospital within the last 30 days or are at high risk of a hospital admission
Has one or more long term medical conditions
Over 50 years old
In need of assistance around the home
Lonely and/ or isolated
