Rowans HospiceRowans Hospice is providing a full range of clinical and support services; within the Hospice, the Living Well Service and to people within their own home.
Rowans Living Well Service is open 5 days a week (from 10 am - 4 pm), people with a life limiting condition, the carers and those that have been bereaved can ‘drop in’ to the Centre, for support, advice and sign posting; potentially accessing support from the programmes of care that are on offer. A communal café provides snacks and a small selection of hot meals within the Centre. Rowans Hospice offers palliative and end of life care to people affected by life-limiting illnesses, to include family members, carers and friends, throughout Portsmouth and South-East Hampshire within its building and to people within their own home, to include specialist multi-professional advice; and reablement social work and specialist dementia support in Portsmouth. We value life, quality of living and foster hope, supporting people to maximise their health and wellbeing and plan ahead for their future care needs. Rowans Hospice Veterans ServiceWe run a dedicated support service for Serving and ex-Service Personnel who are living with or caring for someone with a serious or life-limiting illness, or are recently bereaved.
Did you serve at least 1 day in the Armed Forces or Merchant Navy? Are you experiencing loneliness or social isolation? Are you struggling with the impact of your illness physically and/or emotionally? Have concerns about the future and whether your loved ones will be looked after? Our wide range of services, including a dedicated Veteran Companions buddy service, can help you find answers. Weekly social groups take place on Mondays - Living Well Centre - ring to book - limited spaces ** NEW** Thursdays Gosport Community Hub 10:30 to 12:30 every week - drop in for any veterans and Serving Personnel, who are living with a serious or life-limiting illness where you can seek advice, support or just a friendly chat with others in the same situation. Specialist Palliative Care Clinical Psychology ServiceWhat is palliative care?
Palliative care aims to improve the quality of life for patients who are living with a life shortening illness. It also helps their carers and those close to them. Palliative care is available at any point during a life shortening illness, not just when patients are nearing the end of their life. The palliative care approach is used for all on-going life-shortening conditions, not just cancer. Palliative care is provided at two distinct levels of care: those providing the day-to-day care to patients - such as General Practitioners (GP) and Community Nurses - and those who specialise in palliative care - the Specialist Palliative Care Team (SPCT). The SPCT aims to provide advice and support to patients, family, friends and carers. They also provide advice and liaise directly with the GP and other professionals involved in the patient’s care (such as the District Nurses). The multidisciplinary team (who all have advanced or specialist palliative care skills) consists of: - Consultants in palliative medicine, Clinical Nurse Specialists, Specialist Physiotherapists, Specialist Occupational Therapists, Clinical Psychologists, Re-enablement Social Worker. The SPCT also hold a weekly Early Palliative Care Clinic for those newly diagnosed as palliative. |
