Solent Mind - Portsmouth Community Peer Recovery Team

Recovering from a period of poor mental health can feel overwhelming as there may be obstacles to navigate and new anxieties before gaining back your independence.

Our Community Peer Recovery Team (or CPRT for short) can offer you the help that you need to set manageable goals, access the support you need and start getting back on your feet.

Our support is special because our staff have their own lived experience of living with a mental health issue. As well as practical advice, we can share our own stories, experiences, provide hope and become a role model in your time of need.

Solent Mind Wellbeing Centre

The Portsmouth Peer Support and Wellbeing service is a new style of service offering peer support for anyone who feels low, worried or hopeless at the PositiveMinds Centre in Portsmouth.

Through 1:1 support and useful workshops, we can help you identify your needs and get you moving towards your goals. We also operate a 'tell us once' system: a chat with us can get you connected to your community and its services, without having to explain yourself again and again. We hope to put you in touch with the right support at the right time.

Simply book in to talk to peer support and wellbeing practitioners. The peer practitioners have lived through difficulties themselves and can relate to how you are feeling. We will work with you to identify goals and walk alongside you in your recovery journey.

Specialist Palliative Care Team

What is palliative care?
Palliative care aims to improve the quality of life for patients who are living with a life shortening illness. It also helps their carers and those close to them. Palliative care is available at any point during a life shortening illness, not just when patients are nearing the end of their life. The palliative care approach is used for all on-going life-shortening conditions, not just cancer.
Palliative care is provided at two distinct levels of care: those providing the day-to-day care to patients - such as General Practitioners (GP) and Community Nurses - and those who specialise in palliative care - the Specialist Palliative Care Team (SPCT). The SPCT aims to provide advice and support to patients, family, friends and carers. They also provide advice and liaise directly with the GP and other professionals involved in the patient’s care (such as the District Nurses). The multidisciplinary team (who all have advanced or specialist palliative care skills) consists of: - Consultants in palliative medicine, Clinical Nurse Specialists, Specialist Physiotherapists, Specialist Occupational Therapists, Clinical Psychologists, Re-enablement Social Worker. The SPCT also hold a weekly Early Palliative Care Clinic for those newly diagnosed as palliative.
