New Foundations Community Chaplaincy (NFCC)

New Foundations works across Hampshire to provide comprehensive support for prison leavers of all ages. Returning to a stable and productive life after prison is often an overwhelming challenge. OUTpatients - The UK's LGBTIQ+ Cancer Charity

OUTpatients is the UK’s only LGBTIQ+ cancer charity. We’re proudly led by and for LGBTIQ+ people affected by cancer, and work hard to support and advocate for our community.

Our goal is to help patients, survivors, partners and caregivers feel welcome and supported in cancer services. Through peer support, education, and bespoke resource production, it is our mission to transform cancer care into a more inclusive environment for everyone.

Portsmouth Rehabilitation and Reablement Team (PRRT)

The Portsmouth Rehabilitation and Reablement Team is an integrated health and social care team the purpose of which is to provide responsive support for people whose needs have intensified, often as the result of an acute illness.

Solent NHS Trust is the lead for this service on behalf of both Solent and Portsmouth City Council

The service is available to adults who are registered with a Portsmouth City GP.

Portsmouth Wellbeing Service

Portsmouth Wellbeing Service supports Portsmouth residents to quit smoking, drink less alcohol and achieve a healthy weight through eating well and moving more.

The Wellbeing Service can only support residents of Portsmouth who live within PO1-PO6.

Supports: 1-2-1 and groups.

Rebound

Rebound Group is a confidential, empathetic support group, caring about carers.

We offer support and assistance in both group and one-to-one counselling settings, as needed.

The service is provided free of charge to carers and family members affected by a loved ones' drugs, alcohol and substance misuse and their associated mental health problems.

The group is run by members who have attended the group for many years, and all have first hand experience of addiction.
