Talking Therapies Portsmouth is for everyone aged sixteen or over, who live in Portsmouth and are registered with a GP in the city. You can self-refer by calling us on 0300 123 3934 or by completing the secure form online. https://www.talkingtherapiesportsmouth.nhs.uk/ Once we receive your referral, we’ll contact you within five working days to arrange assessment. We’ll send you some information about the assessment, and a couple of questionnaires to complete before we talk. These questionnaires will help us get a clearer understanding of your difficulties. During the assessment, we’ll ask you how you’re feeling and how you’ve been coping up to now. We’ll listen to you and we won’t judge what you say. It’s ok if you don’t know where to start. Sometimes it’s difficult to describe our thoughts or feelings. We’ll help you make sense of things. We’ll ask a series of standard questions to help us understand the type of support that would be best for you. Nelson Drive

The Parenting NetworkThe Parenting Network is a non profit charitable organisation working to empower parents and build a supportive community of families who live within the city of Portsmouth.
We are a befriending service who actively listen to parents within our communities, providing advice, support and signposting towards more appropriate specialist services. Our work with families is delivered through a series of engaging programmes that promote fun learning, strengthening family bonds and bringing people together whilst also tackling stigmas, gender stereotyping and social isolation. The YOU Trust Social Prescribing PortsmouthWe have a number of teams working with GP Surgeries specialising in Social Prescribing, Health Coaching, and Care Coordination. We work with people to find out what is important to them, linking them with all the great things they want to do that will have a positive impact on their physical and mental health.
The YOU Trust employs a range of skilled workers for our Social Prescribing. Such as: Social Prescribing Link Workers Care Coordinators Health Coaches They are employed on behalf of some innovative GP Practices. These are based inn Portsmouth & South East Hampshire. However, other GP practices employ their own Social Prescribers directly. The YOU Trust - Circles of SupportWe work with people to identify and build their circle of support, which may be friends, family, colleagues, or supporters who form a group, supporting the person to make their own decisions about their life. YOU works in partnership with The EC Roberts Centre and HOPE Portsmouth to support people in crisis in our community.
The Project promotes independence, resilience and recovery from the impacts of COVID through developing Community Circles of Support. We have three Community Circles of Support Partners who help people through outreach, food banks, community larders/pantries and community meals across Portsmouth. The Community Circles of Support Partners will undertake a holistic assessment identifying skills, potential, root causes of crisis, actions the clients want to take and identify strategies to help themselves for future situations. The aim of Community Circles of Support Partners is to help people in hardship, including those with complex needs such as mental ill-health, domestic abuse and social isolation. They will benefit from improvements to their health, wellbeing, skills and independence and to understand how to take personal control of their situation. |
The YOU Trust Support and enablement servicesWe offer three types of care and support to meet everyone’s needs:
Homecare/Support at Home – personal care, support with budgeting, housing, social and leisure activities, daily living activities, emotional and behavioural support Community-Based Support – small groups of people are supported to access relevant volunteering, occupational, social, and leisure activities in your community Enablement – time-limited support to learn a new skill in your home or community based on your goals and what you want to achieve Support in supported living accommodation is designed to help individuals live as independently as possible. This while ensuring they receive the necessary assistance around daily living tasks. The level and type of support vary based on individual needs. Such as preferences and goals. Which may include: household management personal care meal planning and preparation medication management social and recreational activities skill development, such as independent living skills, employment and education In 2021 Hampshire County Council awarded us a 5-year contract to manage their Support and Enablement Services. It covers the: New Forest Totton Hedge End Fareham Havant Waterlooville Petersfield Hayling Island Gosport We also offer our services in other areas, like Portsmouth, where local council reffers clients to us. By working alongside our clients, we can help you achieve your goals. And as partners, we make sure you get the right type of support that you need. We provide services that include personal care, a variety of skill development and occupational, social, and leisure activities. We work closely with clients to create person-centered care and support plans. Because of this, our approach is holistic.
