Community Connectors
Details
Community Connectors provide short term support (usually about 6-8 weeks) to help individuals take the first steps towards getting out and about in their community.
Opening Hours:
The Community Connector Service will support you to build your confidence to access the local community. This could be to go to a group, a learning opportunity, a physical activity or to use public transport so you can go out and about more and do the things you like.
The service is run by The Independence and Wellbeing Team, which is part of Adult Social Care at Portsmouth City Council.
We can work with you on a one-to-one basis for 6-8 sessions, which will take place regularly over a short period of time.
A Community Connector will meet with you in a place you feel most comfortable to have a chat to hear about what your interests are and support you to find what may be available locally. They will work with you to make a plan for you to achieve what you would like to do, this could be going along to a group, activity or building confidence to use public transport. If needed, a community connector will accompany you in the first instance to build your confidence so you can then continue independently.
